IMPROVED PERFORMANCE OF BELUN SLEEP PLATFORM USING SECOND-GENERATION ALGORITHM IN DETECTING OSA: INTERIM ANALYSIS IN 48 PATIENTS

      PURPOSE: Belun Sleep Platform (BSP) is a novel neural network-based home sleep apnea testing system utilizing a wearable ring device to detect obstructive sleep apnea (OSA). Its performance can be improved by training the algorithm through continual data collection. The objective of the study is to assess the performance of the second-generation BSP algorithm, which aimed to minimize the overestimation of AHI at the low AHI range seen in earlier studies.
      METHODS: Polysomnography (PSG) studies were performed simultaneously with Belun Ring in individuals who were referred to the the sleep labs of University Hospitals Sleep Medicine Services (Cleveland, OH) for an overnight sleep study. The sleep studies were manually scored using the current American Academy of Sleep Medicine Scoring Manual with 4% desaturation hypopnea criteria.
      RESULTS: A total of 48 subjects (M:F = 17:31) were examined at the University Hospitals Cleveland Medical Center Sleep Labs. Of these, 31% had AHI < 5; 27% had AHI 5-15; 21% had AHI 15-30; 21% had AHI ≥ 30. BMI 38.7 + 10.0. The Belun apnea-hypopnea index (bAHI) correlated well with the PSG-AHI (r = 0.861, P < 0.001). The accuracy, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and areas under receiver operating characteristic curve (AUROC) in categorizing AHI ≥ 5 were 0.792 [95% CI, 0.650-0.895], 0.818 [95% CI, 0.645-0.930], 0.733 [95% CI, 0.449-0.922], 0.871 [95% CI, 0.702-0.964], 0.647 [95% CI, 0.383-0.858], and 0.887 [95% CI, 0.796-0.978], respectively. The accuracy, sensitivity, specificity, PPV, NPV, and AUROC in categorizing AHI ≥ 15 were 0.875 [95% CI, 0.748-0.952], 0.800 [95% CI, 0.563-0.943], and 0.929 [95% CI, 0.765-0.991], PPV 0.889 [95% CI, 0.653-0.986], 0.867 [95% CI, 0.693-0.962], and 0.954 [95% CI, 0.903-1.000], respectively. Compared to the first-generation BSP, the second-generation algorithm demonstrated significant improvement of specificity at the AHI cutoff of 5 with an increase of Youden index from 0.029 to 0.552 [95% CI, 0.094-0.852].
      CONCLUSIONS: In this interim analysis, the second-generation BSP algorithm not only performed well at the AHI cutoff of 15 but also allows sharper differentiation between mild OSA and normal subjects.
      CLINICAL IMPLICATIONS: BSP continues to evolve with enhanced performance in detecting OSA and can potentially be incorporated into clinical practice and sleep medicine research.
